Can you rent or buy a skidsteer driver like a Montana? We have a 750. I bet you can drive a pipe post right through the rotten tie. Clamp or weld your guardrail to the pipe post.
The more difficult way would be to wet saw the concrete around the post. Then drill the rotted wood out with a post hole auger and replace with another wood post.
